How do I obtain
a copy of my health information/ medical records?
During normal business hours,
you may come to the office with identification and sign a HIPAA compliant authorization.
If it is not practical for you to come to the office, you may mail a notarized
HIPAA compliant authorization to us and we will mail to the copies to you or
to whomever you designate. We will be happy to mail you a HIPAA compliant authorization
for you to sign in front of the notary if you need one. If your records are not
within the last 5 years, they may have to be obtained from our storage facility.
Routine requests take approximately 1 - 2 weeks. Requests for patient care are
processed immediately. Routine charges are 20 cents per page.
How do I get forms, such as disability, FMLA, etc., completed by my doctor?
You must either mail or bring into the office the form and a HIPAA compliant
patient authorization so your physician can release the information to the requestor.
We will be happy to mail you a HIPAA compliant authorization for you to sign
in front of the notary if you need one. Most forms require a $10 fee, which is
payable at the time the form is received. The expected turnaround time is 7 -
10 business days.
